Before the semester begins each Delight chapter needs to submit the Informational Contract + Statement of Agreement to the Delight HQ before January 6th. Only one form needs to be submitted from your chapter. After submitting this form we will send you your chapters books and all other needed material for the semester.

*If you are unable to complete every section of this form contact your Regional Director to adjust the deadline. Partially completed forms will not be accepted.
